DHI vs FUE: What's The Difference?
Updated: Jul 27

Introduction
If you've been researching hair transplants, you've almost certainly come across the terms FUE and DHI.
Many patients believe that FUE and DHI are two completely different hair transplant procedures. In reality, DHI is a method of implanting grafts after they have been harvested using FUE. The biggest difference lies not in how the hair is removed, but in how the grafts are implanted into the scalp.
This guide explains what each term means, how they differ, and how your surgeon decides which technique is most appropriate for your individual case.
What is FUE?
FUE Implantation Technique
In a conventional FUE procedure, implantation occurs in two separate steps.
First, the surgeon creates thousands of tiny recipient incisions using instruments such as:
Sapphire blade
Steel blade
Custom-cut blades
Needle (18–21G)
Each incision determines the:
Hair direction
Hair angle
Hair density
Final hairline design
Once all recipient sites have been created, the grafts are inserted into these incisions using fine forceps by the surgeon or an experienced implantation team.

What is DHI?
Direct Hair Implantation (DHI) is a method of implanting hair follicles using a specialised implanter pen.
Instead of placing grafts with forceps after creating recipient sites, the grafts are loaded into an implanter pen and inserted into the scalp in a controlled manner.
DHI therefore describes the implantation technique—not the extraction method.
Are FUE and DHI completely different procedures?
Not exactly.
In modern hair transplantation, grafts are usually harvested using FUE regardless of whether they are later implanted using forceps or an implanter pen.
In simple terms:
- FUE = how the graft is removed.
- DHI = one way the graft can be implanted.
Many clinics market FUE and DHI as completely different procedures, but technically they describe different stages of the same operation.
Advantages of FUE
FUE offers several benefits, including:
- Suitable for most patients.
- Efficient treatment of larger areas.
- Excellent graft survival when performed correctly.
- Well-established and widely used worldwide.
- Allows careful planning of recipient sites before implantation.
Advantages of DHI-style

DHI may offer advantages in selected cases, including:
- Excellent control over graft placement.
- Precise control of angle and direction.
- Particularly useful for carefully planned hairline work.
- Can be advantageous in selected unshaven procedures.
- May reduce the time grafts spend outside the body during implantation.
The most appropriate technique depends on the individual patient and surgical plan.

Which technique gives better results?
There is no single answer.
Excellent results can be achieved with either technique when performed by an experienced surgeon and surgical team.
In reality, the final outcome depends much more on:
- Hairline design
- Surgical planning
- Donor management
- Graft handling
- Angle and direction of implantation
- Appropriate patient selection
than on whether the procedure is labelled "FUE" or "DHI".
Which technique do we use at HairMD London?
At HairMD London, we do not believe one technique is automatically superior for every patient.
Instead, we recommend the approach that best suits:
- Your pattern of hair loss
- Donor availability
- Hair characteristics
- Desired hairstyle
- Long-term treatment goals
Every treatment plan is personalised rather than based on a one-size-fits-all approach.
Frequently Asked Questions
Is DHI better than FUE?
Not necessarily. Both techniques can produce excellent, natural-looking results when performed by an experienced surgeon. The best approach depends on your hair characteristics, the size of the treatment area, your styling preferences, and your long-term goals. During your consultation, we will recommend the technique that is most appropriate for your individual case.
Is DHI-style less invasive?
No. Both DHI and FUE are minimally invasive hair transplant techniques performed under local anaesthesia. Recovery time, discomfort, and healing are generally very similar for both methods.
Does DHI-style implantation produce higher density?
Not automatically. Natural-looking density depends on careful surgical planning, graft quality, donor availability, and precise implantation. Both DHI and FUE can achieve high-density, natural results when performed correctly.
Does DHI-style implantation leave scars?
Both DHI and FUE involve tiny extraction sites in the donor area that heal as very small dot scars, which are usually difficult to detect once the hair has grown. Neither technique leaves a linear scar.
Which technique heals faster?
Healing is very similar for both procedures. Most patients return to normal daily activities within a few days, while the small scabs typically disappear within 7–10 days.
Is one technique more painful?
No. Both procedures are performed under local anaesthetic, so the treatment itself is generally comfortable. Mild soreness or tightness may be experienced for a few days afterwards, regardless of the implantation technique used.
Which technique gives more natural results?
Natural results depend far more on the surgeon's experience and artistic planning than on the implantation technique itself. Hairline design, graft placement, angle, direction, and density are the key factors that determine the final outcome.
How do I know which technique is right for me?
There is no one-size-fits-all solution. During your consultation, your surgeon will assess your hair loss pattern, donor area, hair characteristics, and treatment goals before recommending the most suitable approach for achieving the best long-term result.
Is DHI-style more expensive?
Costs vary between clinics and usually reflect the complexity of the procedure and the treatment plan.
Which technique has the faster recovery?
Recovery is generally similar, although healing varies from one individual to another.
Key Takeaways
✔️ FUE describes graft extraction.
✔️ DHI describes one method of graft implantation.
✔️ They are not direct competitors—they describe different stages of hair transplantation.
✔️ Excellent results depend on surgical planning, graft handling and experience rather than marketing terminology.
✔️ The best technique is the one that is most appropriate for your individual needs.
